Description
Provides case/care management, supported employment services that include job coaching and placement, social and recreational opportunities and ongoing assessment and evaluation. May also provide assistance with the activities of daily living, including personal care, respite care, and transportation. Also provides Medicaid Waiver, a community-based, one-to-one, long term care services program to individuals with traumatic brain injury.
