Description
Provides in-home visitation by a registered nurse to women during pregnancy and following delivery for a mutually agreed upon period of time. Services provided to the family during this time are decided upon in partnership with the family and are particular to each family's needs and desires. Services may include healthy pregnancy information, prenatal nutrition counseling, early childhood development information, parenting education, home safety information, as well as referrals to healthcare, childcare, continuing education, job training, and other community support services.

The Visiting Nurse Association & Hospice of the Southwest Region is a non-profit, Medicare-certified, home health care and hospice agency delivering services to residents of Southwestern Vermont. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families.
