Description

Provides acute and chronic health care, routine physical exams, certain immunizations, physical therapy and chiropractic services, and prescription assistance. The program also provides limited mental health counseling, lab and x-ray, and healthcare case management. Provides patient referrals to other health and non-health services, and has an insurance assister on staff who can help enroll patients and community members into an insurance plan through Vermont Health Connect. Located within the offices of Four Seasons Dermatology and Vermont Center for Cancer Medicine
