Provides care coordination to individuals around mental health support, substance use treatment, Veterans’ benefits and housing assistance, among other supports. The Veteran Care Navigator program is equipped to help individuals and families with a wide range of intricate challenges including the lingering effects of trauma, service-related injuries, difficulty accessing resources, and stress and emotional burden carried by family members and caregivers, both during and after an individual is deployed.

CCN provides acute and long-term behavioral health care services to people who experience a wide range of behavioral and mental health challenges.
