Description
Provides a full range of supportive services for individuals with a limited life expectancy and their families. Also provides comprehensive palliative and support care to individuals in need of pain and other symptom management, whether or not they have a life limiting illness. Provides bereavement support services that include individual grief counseling and/or support groups. Hospice team works together with a provider to meet the needs of patients and their families. They are not a crisis line but will act as a resource to the community for bereavement issues. The Bereavement support group is a hybrid group. People can attend in person or virtually.

The UVM Health Network Home Health & Hospice is a non-profit, Medicare-certified, home health care agency delivering services to residents of Chittenden and Grand Isle Counties. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families. Services include Home Care, Hospice Care, Palliative Care, Long Term Care, Rehabilitative Therapies, Maternal Child Health Services, and Pediatric Nursing.
